As a rigorous part of the selection procedure, this document is designed to vet candidates through a polygraph examination, ensuring they meet the high standards of integrity and honesty expected of law enforcement officers. Assigned to a specific examiner and accompanied by a unique tracking number, the form begins with basic identification questions, extending to detailed inquiries about personal history, citizenship, and residency. The booklet emphasizes truthful disclosure across various aspects of an applicant’s background, including but not limited to past employment, identity verification, legal encounters, and any history of eviction or financial instability. Completing this form truthfully is paramount, as omissions or falsehoods can lead to disqualification or future termination. Moreover, it includes a section where applicants must provide written permission for the examination, underscoring the voluntary nature of proceeding with the polygraph test. You are being considered for employment with the Maryland State Police. As a prerequisite for employment, you are required to submit to a pre-employment polygraph examination. As part of that process, it will be necessary for you to complete this booklet. The contents of this booklet are held "CONFIDENTIAL," except when essential to the conduct of proper official police business and national security. Maryland State Police employees, regardless of position, are placed in a position of trust and responsibility. Because of this trust, the MSP expects candor and honesty from all employees. The purpose of this examination is to determine the truthfulness of the information provided to the polygraph examiner. The areas of inquiry will deal with your background and experience.

Common mistakes

When completing the Maryland Polygraph Police form, accuracy and thoroughness are critical to ensuring the integrity of the application process. Here are ten common mistakes to avoid:

  1. Failing to read the instructions carefully. This can lead to misunderstanding the requirements of the form.
  2. Leaving blank spaces. Every question should be answered fully; if a question does not apply, indicate with “N/A” (not applicable).
  3. Providing incomplete information, particularly with the personal background section. Full disclosure is vital.
  4. Misunderstanding the importance of the confidentiality agreement and the implications of the information provided during the polygraph examination.
  5. Using nicknames or incomplete names. The form requires your full legal name to ensure proper identification.
  6. Omitting previous addresses or failing to list them accurately in the residency section. It’s important to list all places where you have lived as requested.
  7. Incorrectly answering citizenship questions, either by omission or misunderstanding the questions regarding foreign citizenship, allegiance, or work for foreign governments.
  8. Not providing detailed explanations for “yes” responses where written explanations are required. This includes not explaining past legal issues, identity changes, or inconsistencies in personal history.
  9. Forgetting to sign and date the form where required, which could lead to delays in the application process.
  10. Misrepresenting any facts or not disclosing information that could be verified during the polygraph examination.

It is imperative that applicants approach this form with honesty and take the time to review their answers. Ensuring complete and accurate responses helps maintain the integrity of the application process and assists the Maryland State Police in making informed decisions regarding potential employees.

Similar forms

The Maryland Polygraph Police form is similar to several other documents used within various fields and contexts, primarily due to its comprehensive nature of background information gathering and the requirement of truthfulness from the applicant. One such document it closely resembles is a standard employment application form used across many sectors.

Standard employment application forms, much like the Maryland State Police Applicant Polygraph Screening Booklet, require personal background information, employment history, and sometimes, information about education and references. Both forms serve to offer a comprehensive view of the applicant to the reviewing personnel, pertaining to their identity, past conduct, and suitability for the position applied for. Employment applications, however, generally do not delve as deeply into the legal or personal conduct aspects (e.g., questions about citizenship, past criminal activity, or the use of false identities) as the Maryland Polygraph Police form does. The focus on ensuring honesty and integrity is heightened in the polygraph form through its candid disclosure of the polygraph test requirement, underscoring the importance of truthfulness in the application process for a law enforcement position. This approach aligns with the high standards of trust and responsibility expected from those seeking to serve in law enforcement roles.

Another related document is the security clearance application used by government agencies. Similar to the Maryland State Police Applicant Polygraph Screening Booklet, security clearance applications require exhaustive detail about one's personal history, foreign travel, family background, and potential foreign allegiances. The objective here, as with the polygraph screening form, is to thoroughly vet individuals for positions where they will be entrusted with sensitive information. Both documents require applicants to authorize background checks and, in certain cases, to agree to be subject to ongoing monitoring if deemed necessary. The level of detail and scrutiny in these forms emphasizes the premium placed on national security and the protection of classified information. While the security clearance application process includes its own form of background investigation and may also require a polygraph test, both it and the Maryland Polygraph Police form are based on the foundation of vetting for integrity, reliability, and trustworthiness.

Dos and Don'ts

When filling out the Maryland State Police Applicant Polygraph Screening Booklet, it's crucial to approach the document with care and honesty. The process is not just a formality but a pivotal step in evaluating your suitability for a role that demands integrity and trustworthiness. Here are eight recommendations to guide you through this process effectively:

  • Do read all the instructions carefully before you start filling out the booklet. Understanding what is required of you can help prevent any accidental errors or omissions.
  • Don’t rush through the questions. Take your time to consider each question thoroughly and recall any relevant information that can impact your answers.
  • Do answer every question truthfully. The purpose of the polygraph exam is to verify the honesty of your responses. Providing false information can result in disqualification.
  • Don’t omit details that you think might negatively impact your application. Withholding information or not disclosing certain facts can be more damaging than the information itself.
  • Do provide complete and accurate information about your background and experience. Incompleteness can be interpreted as an attempt to hide something.
  • Don’t forget to review your answers before submitting the booklet. Checking your responses allows you to correct any mistakes or add information you might have initially overlooked.
  • Do prepare any necessary documents or information ahead of time that can help you accurately complete the booklet. This might include dates of previous employment, addresses, and other personal history details.
  • Don’t sign the consent form without understanding the implications. Remember, the polygraph exam and your consent are legally binding elements of the employment screening process.

By following these dos and don'ts, you're not just complying with the procedure; you're demonstrating your ability to approach tasks with the seriousness and honesty expected of Maryland State Police personnel. This integrity is not only critical for the formalities of application but is foundational for the responsibilities you hope to undertake.

  • Misconception 4: Past misconduct automatically disqualifies you. It's often believed that any past "bad" behavior or mistakes will disqualify an applicant. The truth is, failure to provide truthful, accurate, and complete information is what leads to disqualification, not necessarily the past actions themselves.

  • Misconception 5: Signing the release form waives all rights. Applicants might be concerned that signing the release form for the examination means waiving all legal rights. While the form does include a waiver for liabilities relating to the exam, it specifically pertains to the examination process and its direct outcomes.

In reality, it covers a broad spectrum of personal and professional topics, including citizenship, residency, and even personal habits.
